Facebook Ads for Med Spas: What a Booked Consultation Really Costs

As a female entrepreneur, you probably get a lot of conflicting information about how you “should” act. Is it OK to be assertive? Or will that make people not like you? Is it OK to be kind, or does that mean people will take advantage of you?

The short answer: the number that matters for a med spa running Facebook and Instagram ads isn’t cost per lead, it’s cost per booked consultation that actually shows up. To work it out, divide your ad spend by the number of consults that were booked and attended. Lead prices vary a lot by market, treatment and offer, so the med spas that win with Meta ads are the ones that control what happens after the click: a specific treatment offer, a short form or landing page, a reply within minutes, a deposit or booking policy that cuts no-shows, and tracking that follows each lead through to a paid treatment.

This post walks through how to measure that properly, what changes the cost of a consult, and how to set up campaigns so the leads you pay for turn into patients on your schedule.

Why cost per lead is the wrong number for a med spa

Most ad reports lead with cost per lead because it’s easy to see inside Ads Manager. But a lead is just someone who tapped a form. For a med spa, a lead only turns into revenue after several more steps:

  • The lead answers your text or call.
  • They book a consultation or treatment.
  • They show up.
  • They say yes to treatment, and ideally come back.

Every step loses people. A campaign with cheap leads that nobody answers can cost you far more per patient than one with pricier leads who book and show. That’s why we judge med spa campaigns the same way we judge coaching campaigns in our guide to what a booked call really costs: follow the money all the way to the appointment.

How to calculate what a booked consultation really costs

You only need three numbers from your own account and calendar:

  1. Cost per lead (ad spend divided by leads).
  2. Booking rate (the share of leads who book a consult).
  3. Show rate (the share of booked consults who turn up).

Then: cost per attended consult = cost per lead Ă· booking rate Ă· show rate.

Illustrative math (example numbers, not benchmarks)

These figures are made up purely to show how the formula works. They are not industry averages or client results.

  • Say you spend $2,000 in a month and get 80 leads. That’s $25 per lead.
  • If 25% of those leads book, that’s 20 consults, or $100 per booked consult.
  • If 75% of booked consults show, that’s 15 attended consults, or about $133 per attended consult.

Now see what happens if you fix follow-up instead of chasing cheaper leads. Same $2,000, same 80 leads, but faster replies lift booking to 40% and a deposit lifts show rate to 90%. That’s 32 booked and about 29 attended, or roughly $69 per attended consult. The ads didn’t change. The system behind them did.

Finally, compare that cost with what a new patient is worth to you. If a typical first treatment plus likely rebooks brings in many times your cost per attended consult, the campaign is working, even if the cost per lead looks “high” on paper.

What changes the cost of a med spa consult

The offer

“Book a free consultation” is the default, and it’s often the weakest option because it gives people no reason to act now. Offers tied to one specific treatment, such as a first-visit price on a named service, a new-client package, or a seasonal event, tend to attract people who already know what they want. Be careful with deep discounts. They can fill the calendar with bargain hunters who never come back, so price your intro offer to bring in the patients you actually want to keep.

Lead form or landing page

Meta’s instant lead forms are quick for people to fill in, which usually means more leads but lower intent. A landing page asks for a bit more effort and tends to bring in fewer, warmer leads. There’s no universal winner. A practical approach:

  • Use an instant form with one or two qualifying questions (which treatment, best time to reach them) if your team can reply fast.
  • Use a landing page with online booking if you want people to choose a time themselves and you have a strong page. If yours isn’t built for that yet, it’s worth looking at a proper website and landing page setup before you scale spend.

Whichever you pick, track booked and attended consults for each, not just leads.

Speed to lead

Someone who fills in a med spa form on Instagram is often browsing several clinics at once. The clinic that replies first, while the person still has their phone in their hand, usually gets the booking. Set up an instant text the moment a lead comes in, followed by a call from your front desk, and keep following up for several days. A CRM such as GoHighLevel can send that first text automatically and remind your team to call, so leads don’t sit in an inbox overnight.

No-shows

Show rate is the step most med spas ignore and it can quietly double your cost per patient. A few things help:

  • A small deposit or card on file for consults, with a clear cancellation policy.
  • Automated confirmation and reminder texts before the appointment.
  • A short “what to expect” message so the visit feels less intimidating.
  • Booking consults soon after the lead comes in, rather than weeks out.

Creative and targeting

Real photos and short videos of your space, your team and the treatment experience tend to build more trust than generic stock images. Keep targeting local and realistic: people within a reasonable drive of your clinic. Before-and-after images and claims about results are tightly policed by Meta’s advertising standards, so check Meta’s rules before you run them.

Meta’s health and wellness restrictions: what med spas should know

Since early 2025, Meta has restricted some advertisers it classifies as health and wellness from sending certain mid- and lower-funnel events (such as leads and purchases) from their websites through the Meta Pixel and Conversions API. If your data source is restricted, campaigns that optimize for website conversions can lose the signals they rely on, and you may lose some conversion-based audiences. Digiday covered how the rollout was announced to advertisers, and Search Engine Land has a clear overview of the health and wellness restrictions.

Whether this affects your med spa depends on how Meta classifies your website, which you can check in Events Manager. If you are affected, it changes how you set up campaigns, for example leaning on instant forms or tracking bookings in your CRM instead of relying on website conversion events. Either way, never send health information about patients through your pixel, forms or ad audiences. This is general information, not legal advice, so check your tracking setup with your compliance advisor.

How much should a med spa start with?

Rather than picking a number out of the air, work backward:

  1. Decide how many new attended consults you want per month.
  2. Estimate your cost per attended consult from your own past data, or start conservatively and measure for the first few weeks.
  3. Multiply the two. That’s your starting monthly budget.

Give a new campaign enough time and budget to collect real data before judging it, and expect the first few weeks to be about learning which offer and creative bring in patients who book and show. We’ll go deeper on budgets in a separate post on what a med spa should spend on ads each month.

A simple med spa campaign setup that works

  1. One treatment, one offer, one audience to start. Don’t promote your whole menu in one ad.
  2. Three to five creatives: a short video from your space, a team or injector intro, a client-experience walkthrough, and a clear offer image.
  3. A short form or booking page with one qualifying question.
  4. Instant text plus a call within minutes, then a few days of follow-up.
  5. Deposit and reminders to protect show rate.
  6. Weekly review of cost per lead, booking rate, show rate and cost per attended consult, so you know which lever to pull.

This is the same disciplined approach we use across all our paid campaigns. If you’re new to running ads properly rather than boosting posts, our guide to Meta ads that bring in clients covers the basics, and you can see how we work on our advertising and paid media page.

FAQ: Facebook ads for med spas

Do Facebook ads work for med spas?

They can, when they’re run as a booking system rather than a likes campaign. A specific treatment offer, local targeting, fast follow-up and a show-rate plan matter more than the ads alone. Judge them on cost per attended consult and new patients, not cost per lead.

How do I calculate cost per consultation?

Divide cost per lead by your booking rate, then by your show rate. For example, with made-up numbers, $25 per lead with 25% booking and 75% showing works out to about $133 per attended consult.

Should a med spa use lead forms or a landing page?

Instant lead forms usually bring more, lower-intent leads, while a landing page with booking brings fewer, warmer ones. Test both and compare booked and attended consults, not lead counts.

How do I reduce no-shows from Facebook leads?

Reply within minutes, book the consult soon, take a small deposit or card on file, and send automated confirmations and reminders. These steps protect your show rate, which directly lowers your cost per patient.

Do Meta’s health and wellness rules affect med spas?

They can. Meta restricts some lower-funnel tracking for websites it classifies as health and wellness. Check your data source category in Events Manager, keep patient health information out of your pixel and forms, and check your setup with a compliance advisor. This is general information, not legal advice.
